The mobility of the lower jaw enables chewing and speaking.  <\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-a7a4518172f08a63234435ebd7ba9354 wp-block-paragraph\">Due to the strong masticatory muscles, the temporomandibular joint is exposed to strong forces, which can lead to mechanical malfunctions and accelerated wear, especially in the case of asymmetries in the jaw and tooth area.<\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-dffebe8f5863aee230639871ac467976 wp-block-paragraph\">With TMJ syndrome, patients notice intermittent pain in the joint in front of the ear or in the muscles around the joint when they speak or chew. In severe cases, patients can no longer open their mouth properly and chewing is no longer possible: food is swallowed unchewed or patients are limited to a liquid diet. <\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-d4fce5c5fa2e8181199f8bb694807e1f wp-block-paragraph\">The complex structure and function of the temporomandibular joints is often poorly understood, even by doctors. The joint has four main components: Bone (with cartilage), discus, muscles and ligaments. This articular cartilage ensures that the joint glides smoothly and acts as a protective cushion for the underlying bones where they meet as a joint.<\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-e598bd75a2b299ef3e01713d08ab324e wp-block-paragraph\">In addition, as in the knee, there is a natural spacer in the joint called the &#8220;discus articularis&#8221; (articular disk of the temporomandibular joint). This is a structure similar to the &#8220;meniscus&#8221; in the knee, which moves with the joint-forming head of the lower jaw when the jaw is opened and closed and helps to evenly transfer the immense loads that can occur when chewing, thus protecting the joint.<\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-57d32f7c82eeccfb0837c36f6bdc42d4 wp-block-paragraph\">As with any other joint in the body, the cartilage and\/or the disc in the temporomandibular joint can be injured or damaged by wear and tear, leading to painful signs of wear and tear &#8211; osteoarthritis.<\/p>\n\n<\/div>\n<\/div><\/div>\n\n<div class=\"gb-grid-column gb-grid-column-3dff5db5\"><div class=\"gb-container gb-container-3dff5db5\">\n<div class=\"gb-container gb-container-db34a62d\">\n\n<h5 class=\"gb-headline gb-headline-5b4669d4 gb-headline-text\">Treatment options<\/h5>\n\n\n\n<p class=\"has-main-turq-hover-color has-text-color has-link-color wp-elements-209bec9c558d2513d1e38790e6fbeed9 wp-block-paragraph\" style=\"font-size:16px\">While joint replacement with an endoprosthesis (artificial joint) is now standard for other joints, this is difficult for the temporomandibular joint and is associated with unsatisfactory functional results.<\/p>\n\n<\/div>\n<\/div><\/div>\n<\/div>\n<\/div><div class=\"gb-container gb-container-40bdefe9\" data-aos=\"fade-in\" data-aos-anchor-placement=\"top-bottom\">\n<div class=\"gb-grid-wrapper gb-grid-wrapper-37703930\">\n<div class=\"gb-grid-column gb-grid-column-67f9a325\"><div class=\"gb-container gb-container-67f9a325\">\n<div class=\"gb-container gb-container-45ba3cb2\" id=\"txt\">\n\n<h3 class=\"gb-headline gb-headline-7d861a05 gb-headline-text\">Stem Cell Therapy for Temporomandibular Joint Arthrosis<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">One modern treatment option for temporomandibular joint degeneration is the use of stem cells. These can regenerate cartilage and the articular disc, or at least slow down the progression of osteoarthritis so that joint replacement can be delayed.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In cooperation with the ANOVA Institute for Regenerative Medicine, stem cell treatments of temporomandibular joints can be carried out. ANOVA IRM is the first and currently the only institution in Germany to have an official license for the production and treatment of corresponding stem cell products.<\/p>\n\n<\/div>\n<\/div><\/div>\n<\/div>\n<\/div><div class=\"gb-container gb-container-7b2744b5\" data-aos=\"fade-in\" data-aos-anchor-placement=\"top-bottom\">\n<div class=\"gb-grid-wrapper gb-grid-wrapper-be487ac3\">\n<div class=\"gb-grid-column gb-grid-column-7d2a3442\"><div class=\"gb-container gb-container-7d2a3442\">\n<div class=\"gb-container gb-container-5240054b\" id=\"txt\">\n\n<h3 class=\"gb-headline gb-headline-b490907c gb-headline-text\">Diseases of Muscles and Ligaments<\/h3>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-a763917314887a0df26d9a6047026e95 wp-block-paragraph\">Another important component of the temporomandibular joint are the muscles that move it. When chewing, these muscles have to work together with millisecond precision to enable the teeth to open and close and make lateral grinding movements. Overloading or incorrect loading can disrupt the biomechanics of the muscles, for example through muscle cramps &#8211; permanent contractions of muscle parts, or through muscle fiber tears and associated inflammation. These dysfunctional muscle parts are also called &#8220;trigger points&#8221;. As in other muscles, these changes are associated with considerable pain.  <\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-8841566a1f01cd26a03fe817ef341729 wp-block-paragraph\">The tendons through which the muscles attach to the corresponding bones can also develop tears, degenerative changes and inflammation due to overloading and incorrect loading, which can lead to pain and impaired biomechanics.<\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-779d949b71b6e04c5fe561f42802ebd7 wp-block-paragraph\">Ligaments stabilize the joint by guiding it and preventing excessive movements for which it is not designed. The temporomandibular joint has many ligaments that are often not taken into account in the treatment of TMJ disorders.<\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-760f5865a3530a0085a6dfa3ff44b857 wp-block-paragraph\">The last two key components, the muscles and ligaments, are often overlooked by doctors, even though they are the key to successful treatment of TMJ disorders.<\/p>\n\n<\/div>\n<\/div><\/div>\n\n<div class=\"gb-grid-column gb-grid-column-72cad7d6\"><div class=\"gb-container gb-container-72cad7d6\">\n<div class=\"gb-container gb-container-8b73917c\">\n\n<h5 class=\"gb-headline gb-headline-b69db819 gb-headline-text\">Treatment Options<\/h5>\n\n\n\n<p class=\"has-main-turq-hover-color has-text-color has-link-color wp-elements-b5ed1064234e2e1ef50e452aca6cd32a wp-block-paragraph\" style=\"font-size:16px\">Targeted physiotherapeutic measures can work wonders for the temporomandibular joint. This first requires a detailed diagnosis, which is carried out using dynamic MRI and a dental examination of the temporomandibular joints, the upper and lower jaw and the teeth.<\/p>\n\n\n\n<p class=\"has-main-turq-hover-color has-text-color has-link-color wp-elements-d9c59e5f76f31dce4dc1c6ecf1c9a24e wp-block-paragraph\" style=\"font-size:16px\">Phasiotherapeutic treatment should then be carried out by an expert specializing in the temporomandibular joints in order to bring about rapid relief of the symptoms and improvement in function.<\/p>\n\n<\/div>\n<\/div><\/div>\n<\/div>\n<\/div><div class=\"gb-container gb-container-a89052f2\" data-aos=\"fade-in\" data-aos-anchor-placement=\"top-bottom\">\n<div class=\"gb-grid-wrapper gb-grid-wrapper-edcbdaa8\">\n<div class=\"gb-grid-column gb-grid-column-044a344b\"><div class=\"gb-container gb-container-044a344b\">\n<div class=\"gb-container gb-container-f91bf3fb\" id=\"txt\">\n\n<h3 class=\"gb-headline gb-headline-03c0ba99 gb-headline-text\">Stem Cell Therapy for Arthritis of the Temporomandibular Joint<\/h3>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-ad12347454befa447b3eddb286503ef3 wp-block-paragraph\">Muscles, vision and ligaments can also benefit from stem cell treatment. In contrast to X-rays and computed tomography (CT), which only depict the bony structures of the TMJ, MRI provides comprehensive information about all anatomical elements of the TMJ. Particularly important in diagnostics are the imaging of the articular disc, ligaments, joint capsule, as well as the muscles and tendons of the TMJ.<\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-9667ca1d1cf5803af6e315386b965121 wp-block-paragraph\">However, there are only a few specialists in the world who are able to examine the TMJ with MRI and analyze this complex data. Dr. Markus Sens is one of these experts and has worked intensively on the diagnosis and treatment of TMJ over the last few decades.<\/p>\n\n<\/div>\n<\/div><\/div>\n<\/div>\n<\/div><div class=\"gb-container gb-container-be523ebb\" data-aos=\"fade-in\" data-aos-anchor-placement=\"top-bottom\">\n<div class=\"gb-grid-wrapper gb-grid-wrapper-8f9ed367\">\n<div class=\"gb-grid-column gb-grid-column-a04ae53e\"><div class=\"gb-container gb-container-a04ae53e\">\n<div class=\"gb-container gb-container-c56cfe7a\" id=\"txt\">\n\n<h2 class=\"gb-headline gb-headline-f5d1e799 gb-headline-text\">Dynamic MRI of the Temporomandibular Joints<\/h2>\n\n\n\n<h3 class=\"gb-headline gb-headline-71115dd3 gb-headline-text\">State-of-the-art diagnostics show pathological functional states of the temporomandibular joints in movement studies<\/h3>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-a6ce2abb219181b6b565292f395724fb wp-block-paragraph\">However, MRI makes even more possible. Using dynamic examination techniques, the complex movement of the TMJ when opening and closing the mouth can be visualized in real time. In particular, the movement of the discus articularis, which is often malformed and displaced in diseases of the TMJ, can be assessed. <\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-104663a4691e7936087a0b9bca2fa93a wp-block-paragraph\">Figure a) shows an example of such a dynamic MRI. The MRI images were taken in close temporal sequence when opening and closing the mouth. The movement study shows in detail the forward displacement of the temporomandibular joint head from the articular fossa onto the articular tubercle, whereby the sliding movement is accompanied by the articular disc. Overall, the mouth opening normally represents not only a rotation in the TMJ, but also a forward translation. This is what makes an effective grinding movement of the teeth possible in the first place.<\/p>\n\n<\/div>\n<\/div><\/div>\n<\/div>\n<\/div><div class=\"gb-container gb-container-41fc5ec5\" data-aos=\"fade-in\" data-aos-anchor-placement=\"top-bottom\">\n<div class=\"gb-container gb-container-96f3f9ef\">\n<div class=\"gb-grid-wrapper gb-grid-wrapper-ab2f6c1a\">\n<div class=\"gb-grid-column gb-grid-column-b504ea18\"><div class=\"gb-container gb-container-b504ea18\">\n\n<p class=\"gb-headline gb-headline-f8a7acf2 gb-headline-text\"><em>Dynamic MRI of the left temporomandibular joint shows the complex forward and backward movement of the mandible with the condyle and the articular disc in and out of the socket.<\/em><\/p>\n\n<\/div><\/div>\n\n<div class=\"gb-grid-column gb-grid-column-29261e39\"><div class=\"gb-container gb-container-29261e39\">\n\n<figure class=\"wp-block-video\"><video height=\"464\" style=\"aspect-ratio: 464 \/ 464;\" width=\"464\" controls=\"\" src=\"https:\/\/bilddiagnostik.de\/wp-content\/uploads\/2023\/10\/kiefergelenke-Film_1.webm\"><\/video><\/figure>\n\n\n\n<figure class=\"gb-block-image gb-block-image-7d74fcea\"><img decoding=\"async\" width=\"653\" height=\"664\" class=\"gb-image gb-image-7d74fcea\" src=\"https:\/\/bilddiagnostik.de\/wp-content\/uploads\/2024\/06\/c-1.jpg\" alt=\"\" title=\"c\" srcset=\"https:\/\/bilddiagnostik.de\/wp-content\/uploads\/2024\/06\/c-1.jpg 653w, https:\/\/bilddiagnostik.de\/wp-content\/uploads\/2024\/06\/c-1-295x300.jpg 295w\" sizes=\"(max-width: 653px) 100vw, 653px\" \/><\/figure>\n\n<\/div><\/div>\n<\/div>\n<\/div>\n<\/div><div class=\"gb-container gb-container-6a31311f\" data-aos=\"fade-in\" data-aos-anchor-placement=\"top-bottom\">\n<div class=\"gb-grid-wrapper gb-grid-wrapper-66484557\">\n<div class=\"gb-grid-column gb-grid-column-7f99ee6b\"><div class=\"gb-container gb-container-7f99ee6b\">\n<div class=\"gb-container gb-container-6d7c28a7\" id=\"txt\">\n\n<h3 class=\"gb-headline gb-headline-970c71bf gb-headline-text\">Stem cell therapies enable pain reduction, improved function and regeneration of the temporomandibular joints without surgery<\/h3>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-4f4d9c36c8f875a4b3f67ec2ea95a821 wp-block-paragraph\">In the meantime, numerous scientific studies have shown that stem cell therapies can be successfully used to treat temporomandibular joint wear &#8211; also known as osteoarthritis. This is not really a surprise, as stem cells have also been used successfully for years to treat osteoarthritis in other joints.<\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-3acce8c7d0f4e16e54b0af09fccc2a67 wp-block-paragraph\">In a study published in 2020 from the Klinikum Hamburg Eppendorf, German scientists were able to show that mesenchymal stem cells (mesenchymal stromal cells &#8211; MSC) injected into arthritic temporomandibular joints, together with or without hyaluronic acid, had a statistically significant positive effect on the regeneration of articular cartilage in the TMJs treated with them. Notably, a significant regeneration of the joint cartilage could be detected after a single application.<\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-cdeef149e902b212f851c742b2d75f17 wp-block-paragraph\">However, not all stem cells are the same. So far, mesenchymal stem cells, whose anti-inflammatory and regenerative effects have been scientifically proven in animal models as well as in initial clinical studies, have been the most frequently investigated.<\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-893f6c10ba45b915daf8bf0d056c4f27 wp-block-paragraph\">Bone marrow stem cells, which can be obtained as a bone marrow concentrate from the pelvic bone, have also been used successfully for years to treat osteoarthritis, e.g. of the shoulder, hip and knee joints. Recent studies show that they are also effective in osteoarthritis of the TMJ.<br\/>As early as 2017, Zaki and colleagues, among others, were able to show that bone marrow stem cells can lead to extensive freedom from inflammation and normalization of the TMJ in cases of experimentally induced arthritis of the temporomandibular joints in experimental animals. The stem cells are stimulated in the laboratory, outside the body, to secrete large quantities of their secretome. This secretome is the active ingredient of stem cells, with the help of which they exert their anti-inflammatory and regenerative effect. The secretome contains hundreds of bioactive substances, cytokines and microRNAs through which the stem cells act. The original idea that stem cells applied to the body replace damaged or dead cells in the body is now outdated.<\/p>\n\n\n\n<p class=\"has-contrast-2-color has-text-color has-link-color wp-elements-482e45e6ea7f774fd7f152ff0581fee6 wp-block-paragraph\">The advantage of treatment with stem cell secretome is that it is cell-free and therefore easy to store.
